Overview
The regional medical environment surrounding cardiovascular medicine is facing a significant transformation, with the differentiation of hospital functions and the development of medical ICT (Information and Communication Technology). There is a need for a new cooperative system of cardiovascular medicine that can respond flexibly and effectively to complex conditions and must be sustainable.
Among cardiovascular diseases, the number of patients with heart failure is increasing rapidly, and we consider it extremely important to prevent the onset and worsening of heart failure. In cardiovascular medicine, treatment options such as medical drugs and devices are increasing and being updated. Therefore, it is necessary to collect and analyze detailed information (clinical images, physiological tests, blood tests, and medical reports) and treatment information in real-time as much as possible and provide timely feedback. In addition, there is a need to build a system that supports patients in the community to deal with problems such as the comorbidity and aging of heart failure patients. In addition, from the perspective of heart failure management, the need for home medical care is increasing, and remote monitoring and daily self-management (PHR: Personal Health Record) of patients become important approaches. Therefore, we aim to collect data of heart failure patients in Miyagi Prefecture and Tohoku University, where the population is aging, and to build a new heart failure treatment model in Japan.

Content of the project
In this project, we plan to use existing systems for specimen test data, findings reports, and medical history (heart failure format). We also carry out exploratory studies on new collection systems for clinical image data and physiological test data. We aim to develop a foundation for a "cardiovascular integration platform" that contributes to feedback and regional cooperation. Detailed real-world data is expected to be used for research and education in terms of visualizing and analyzing the course and pathology of individual cases.

概要：
循環器医療を取り巻く地域医療環境は、病院機能の分化、医療ICT化推進、遠隔診療の導入など大きな変革期を迎えています。複雑化するニーズに対して柔軟かつ効果的に対応しかつ持続可能な循環器医療の新たな連携システムが求められています。
循環器疾患の中でも「心不全」の患者数が急激に増加しており、その発症および重症化の予防は極めて重要です。心不全診療では、薬物に加え非薬物（デバイス）等の選択肢が増え新規治療法の開発も盛んに行われています。詳細な検査情報（臨床画像・生理検査・検体検査・検査レポート）・治療情報をリアルタイムで収集・解析してタイムリーに臨床現場にフィードバックすることが求められています。心不全患者では高齢化と併存疾患の多様化という背景があることかた、患者を地域で支えるシステムの構築が課題です。心不全管理という観点からも遠隔モニタリングや、PHR：パーソナルヘルスレコードを活用した在宅管理へのニーズが高まっています。高齢化が進む宮城県において、東北大学を中心とした関連病院ネットワークを基礎とした心不全症例のデータ収集と共に本邦での新たな心不全診療モデル構築を目指します。

プロジェクトの内容：
本プロジェクトでは、地域連携に資する「循環器統合プラットフォーム」の基盤を整備することを目指します。検体検査データ、所見レポート、病歴（心不全フォーマット）などの既存システムに、臨床画像データ，生理検査データを探索的に収集します。詳細なリアルワールドデータは研究のみならず、個々の症例の経過や病態を可視化・分析するという点で教育面でもその活用が期待されています。
