How to Apply
Applicants are required to submit all the documents in PDF files through their school office.
Due to the unstable acceptance situation caused by COVID-19, we will ask the applicants to prepare the documents in two steps for a while. That is, applicants have to submit the CV first, and once the departments are fixed tentatively, applicants are required to submit the other documents for official acceptance.
Step 1: First application
Application document (Deadline: 5 months before the training)
(1) Curriculum Vitae of the applicant (free format)

- The school office should send us the applicant’s CV
- Please inform the list of the department (at least 2) and rotation term the students wish.

(2) Application form (Download)
(3) Recommendation Letter from the supervisor at your school (free format)
(4) Certificate of enrollment
(5) Academic transcript
(6) Valid passport
Step 2: After departments for training are tentatively fixed
Application documents (Deadline: 4 months before the training)
(7) Chest X-ray findings (taken within 6 months before the traveling date)
(8) Certificate of vaccination record (should be certified by the medical facility or government. Free
format):
- BCG, Measles, Mumps, Rubella, Varicella, Hepatitis B, and COVID-19
*Immunization guide for visiting students
(9) Antibody titer within the past 5 years: Measles, Mumps, Rubella, Varicella and Hepatitis B
- *In case you cannot reach the facility where can perform the antibody titration, or the titer was
negative/equivalent, additional boosters are required. If such a case, submit the certificate of
booster shots also.
(10) Travel insurance covering entire travel (should include at least medical treatment (Injure, illness)
and personal liability)
